Submit your nominations and applications for the 2018 Circle of Life Awards by August 14, 2017. The award honors innovative programs in palliative and end-of-life care in hospices, hospitals, health care systems, long-term care facilities, and other direct service settings. Shine a light on programs and organizations that can serve as models or inspiration for other providers. The award, which is administered by the Health Research & Educational Trust of the American Hospital Association, is cosponsored by NASW.
