JOB OPENINGS: Adventist Health White Memorial Hiring For Two Positions in Los Angeles

Adventist Health White Memorial (AHWM) is a 501(c)3 with more than 150,000 patient visits in 2019 in a community comprised of 91.1% Hispanics. We are a 353-bed not-for-profit, faith-based, academic teaching hospital that provides a full range of inpatient, outpatient, emergency and diagnostic services to Boyle Heights, East Los Angeles, and the Greater Los Angeles communities. Nationally recognized forexceptional services, winning the prestigious Malcolm Baldrige National Quality Award in 2019 as well as the American Hospital Association’s Foster G. McGaw National Prize for Excellence in Community Service seeks two board Licensed Clinical Social Workers.

  1. We’re looking for someone to join our team as a Homeless Navigator, Licensed Clinical Social Worker (LCSW) who will be dedicated to serving the homeless in our community and working exclusively with our homeless patients. During this pandemic, this role will be essential in focusing on helping the homeless who are at risk for COVID-19 both inpatient and out in the community. Bilingual English/Spanish
  1. We’re looking for someone to join our team as a Palliative Care, Licensed Clinical Social Worker (LCSW) who will be dedicated to serving the inpatient palliative care patients. This position will require a highly driven, passionate, self-motivated, and thorough person that is bilingual Spanish/English who will serve to coordinate the inpatient palliative care program, develop and report quality metrics, and facilitate implementation of evidence-based best practices to promote excellent outcomes for patients and families. 
    Essential Functions:
    1. Development and implementation of evidenced-based Palliative Care program
    2. Ability to measure and report quality metrics related to palliative care team performance
    3. Collaboration and communication with multiple disciplines
    4. Collaborates and facilitates patient discharge plans for those requiring post Hospice or post inpatient care
    5. Assists patient/family/caregiver in determining appropriate level of care
    6. Initiates a plan of care and appropriate patient teaching. Reassesses plan of care according to the physical and emotional needs of patient.
    7. Develops social work Plan of Care (POC) based on patient assessment including, psychosocial factors that would impede the POC, clinical knowledge of appropriate theories of stages of human development, the impact of terminal illness, mental status and family dynamics. Documents relevant data accurately and in compliance with laws and regulations
    8. Actively participates in training and/or cross-training of duties
    9. Provide a high standard of holistic care encompassing physical, psychological, spiritual and social care to patients and families
    10. Offers specialized knowledge and skill in the care of terminally ill patients and their families
    11. Disseminates specialist knowledge to hospital associates and members of the primary health care teams
    12. Performs other job-related duties as assigned
