Job Opening: Clinical Social Worker in Sacramento County

Job Description and Duties

California Corrections Health Care Services seeks to fill a Permanent/Full-Time Clinical Social Worker (Health Facility) position in the Whole Person Care Program (WPCP) within the Nursing Services.

Under the supervision of the Headquarters Chief Nurse Executive (CNE) of the Whole Person Care Program (WPCP), Supervising Psychiatric Social Worker I, or designee, the Primary Care Resource Social Worker (PC Resource SW) organizes and provides clinical social work services for patients with chronic diseases such as but not limited to: Substance Use Disorder (SUD), Hepatitis C (HCV), Human Immunodeficiency Virus (HIV) as part of the WPCP/Integrated Substance Use Disorder Treatment and other programs in accordance with applicable policies, procedures, regulations, and program requirements in a variety of correctional health care settings. The PC Resource SW serves as an adjunct member of the Primary Care Team (PCT) and collaborates with the PCT, Addiction Medicine, Mental Health, the Department of Rehabilitative Programs (DRP) and the Division of Adult Institutions (DAI), to screen, assess, recommend level of care, determine individual treatment needs, formulate treatment goals, and plan for disposition of patients. The PC Resource SW implements the appropriate treatment plan and collaborates with internal and external partners to ensure appropriate linkage of resources to ensure effective adjustment into correctional setting and appropriate disposition from programs and/or effective community re-entry.
