The Alzheimer’s Association, California Southland Chapter, is seeking a Care Consultant in the Los Angeles Area Office. This position supports caregivers/families on issues related to care giving and dementia. Person will provide: telephone screening/ assessment of caller’s needs, develop care plans for families; referrals to services and conduct follow-up regularly; services as needed.
